For all appointments call (03) 9347 6301

We are open Monday to Thursday (except Holidays) from 9:00am to 5:00pm, and Fridays from 9:00 am to 4:00 pm. As a Specialist clinic, patients need referral letter from their General Practitioner or referring physician in order to receive a Medicare rebate.

Please arrive 5-10 minutes early for your appointment to fill in a health questionnaire and supply your personal details.

When you come for your appointment, remember to bring the following:

• Medicare or DVA Card
• Your insurance information.
• Copies of operation records, medical records, x-rays, MRIs, CT scans and
  so on from prior doctor visits. For x-rays, please bring the reports and the
  actual films if possible.
• If you have had surgery elsewhere that is related to your current problem,
  please bring a copy of your operation report.
• A list of current medications and any known allergies
  

To cancel an appointment:

Telephone the office during business hours. Please cancel at least 1 day ahead so that your appointment time can be allocated to another patient who is seeking treatment.
